This form should be completed by food business operators either 28 days prior to commencing operations or to amend the details of an existing registration. Unless they benefit from certain exemptions, food business establishments that handle, prepare or produce products of animal origin may need to be approved by the Department, rather than registered, prior to commencing operations. Please contact the Department for guidance if you are unsure whether any aspect of your operations would require your establishment to be approved.
